On page 3 (service details) tick the box next to "I am not represented by a solicitor in these proceedings" and leave the rest of that part blank. On page 10 (part 5 - signature), print your name, and sign it in the appropriate boxes, cross out " (Litigation friend)/(Applicant‘s solicitor)".

Don''t forget to arrange your MIAM and get the mediator to sign the relevant part of the form for the MIAM stuff.

If you have a need to withold your address from the other party, you will first need to complete form C8, you will also need a crime reference number from the police for this application.Make this application first before submitting Form A.

Just in terms of process, I would never recommend assuming Court will correctly sort things out - eg not disclose your address. Always assume that anything you send to court is copied to the other party.
