Form D84 - mark A on the acknowledgment of service

On down d84, it has "I identify the signature appearing in paragraph 9(a) of the acknowledgment of service, a copy of which I have attached to this statement and marked 'A', as the signature of my spouse, who is the respondent in these proceedings"

Where do I mark A on the acknowledgment of service form? Next to the signature? Do I need to write anything else?
