HELP! some legal info needed

I have been divorced for 9 years, I did my own legal work right down to drafting the Consent Order, my ex periodically missed child support payments and from my records it seems I applied for an attachment of earnings Order in 2007, the case resulted in a variation of the Order and he was made to pay a % of the arrears from the original Order, all in all it was a higher amount than the CSA calculation. He has constantly for the past 4 years either missed payments or reduces them depending on how much he feels I deserve and I have finally had enough so need to start a new application. My questions are as follows -

Is is the D50k form?

What case number do I use - I have many different case numbers from different aspects of the divorce and various other hearings. Is it the original consent Order case number or would there have been a new case number issued when the Order was varied (I do not ever remember a replacement order being sent to me after the hearing so unsure what to ask the court for to attach to my case)

I do hope someone knows so I can get on with this after 6 years of being messed around.
