Introduction
An allocation questionnaire is used in finacial remedy proceedings if your case is a very high value case or is a complex case then you can apply for your case to be allocated to a higher court or a more specialised judge by completing an optional Allocation Questionnaire.
The guidelines for ase allocation state that cases should be allocated as follows:
Cases with over £15 million in assets or over £1 million in income are referred to a High Court judge.
Other complex cases should be allocated to a District Judge who is qualified to hear complex cases.
All other cases should be allocated to a disctrict judge.
Who is required to submit an allocation questionnaire
Both parties are required to submit an allocation questionnaire.
When is the allocation questionnaire due
The court will tell you when this is due as it is different for each case.

There will be no costs to fill out the allocation questionnaire.

To fill in section 1 you need to do the following:
Write the applicant's name in the box that says [Applicant]
Write the respondent's in the box that says [Respondent]
Write the date of you got married/ entered a civil partnership in box a.
Write the date you separated in box b.
Write the date the petition was issued on in box d.
Write the name of the divorce centre you use in the blank space below.
Write your case number in the box that says [Case Number]
Write the date your decree nisi/ conditional order was prononuced on in box e.
Write the date your decree absolute/ final order was granted on in box f.
To fill in section 2 you need to do the following:
Write the applicant's FULL name (including middle names) in the box that says 'Full name of Applicant'
Write the respondent's FULL name (including middle names) in the box that says 'Full name of Respondent'
Write down all the details asked of you in the 'Child' boxes of any relevant children, If there are more than 3 relevant children you can you additional pages.
Full name - Child's full name INCLUDING middle names.
Date of birth - The child's date of birth in DD/MM/YYYY format.
Gender - The child's gender.
Relationship to applicant/respondent - Mother, father ect.
Country of residence - Where does the child currently live.
To fill in section 4 you will need to do the following:
Tick the box which shows how much your assets are estimated to be worth. E.g. if your assets are worth under £1 million you'd tick box B.
If you tick box A, B or C you will need to give reasons as to why your case is complex and should be treated as such, and not heard at your local hearing centre.
Write the net value of the family home (after deducting the mortgage) in this box.
Tick the boxes next to any issues that may arise during the financial remedy.
To fill in section 5 you will need to do the following:
If there are any other reasons as to why your case is complex not listed, tick the box.
You will need to give your reasons as to what boxes you have ticked in section 4 (the issues that may arise) and you will also need to explain what other reasons if you have ticked box B.
Put your signature in the 'Signed' box, and write the date in DD/MM/YYYY format in the 'Dated' box.
How to submit your allocation questionnaire
To submit your allocation questionnaire, you will need to file it with the family court that is dealing with your case.
