How to apply for a penal notice on a court order

Hi all
Don't want to bore you all with the details but divorced 10 yrs ago, ex stayed in matrimonial home both parties earn roughly same no other assets, she has repeatedly said she wants me to sign house over I have said I will subject to her getting mortgage and payout of equity about 25k , she refused and filed for court.
She refused to comply with court in terms of peachy paperwork and as she's self employed wouldn't get be accounts or tax liability as said they are confidential, both of us were self representing, at FDR the judge advised her to pay out or we would have to sell and it was very straightforward, she then issued another court order that by may 11th her paperwork had to be in as she only estimated income on form E . The judge rescheduled FDR to give her time to see if she could get mortgage and raise funds.
However instead she has instructed solicitors who have now sent me again the questionnaire slightly reworded and said she will not look at settling if I don't answer even though the judge discounted them off the questionnaire to my ex wife's displeasure, I have responded that I have fulfilled all the court orders and their client hasn't and given a further 7 days before I go back to the court to apply for a penal notice to be attached as this paperwork should have been given in February.
My question is how do I actually apply for this I have had a look and can't seem to see what form I fill out, also does anyone know length of time as the rescheduled FDR is 28th July
