This form would usually be completed by the solicitor you have disinstructed so they can come off the court record. It seems to be a catch all form covering acting as LIP as well as changing solicitor. As such I think you need to complete A, your former solicitor completes B and whichever of you serves the Notice on the other side completes C. C confirms the new address for service of documents.
